EDIT - I had ATI tray tools installed, and it logs the temp readings (I wish it did the clocks though)
And this is what I got -
Also observed a single red LED light on the card with the words "D1601 Overtemp Protect" written on the board near the LED"
Is this a card problem or a PSU?

I have a new Sapphire 4830 card with Catalyst 9.3 on a newly built system (GA-G31M-E2SL, 4gb G.Skill, SATA HDD, SATA DVD optical, e5200, Antec EarthWatts 430W)
In the last week and a half, there have been 4 times when my card, without any warning or symptoms, stopped output and the fan went to 100%. The rest of my system was still operating, causing me to reset/restart the system.
I have not been playing any games, and it has occurred basically while web surfing or desktop publishing. I can not see any links between a similar system state between the 4 outages and they do seem random. The day it started, there were two events, and the other two were spread out. Many time I leave my system on overnight to run folding at home (cpu clients), and it has never happened while I left the computer unattended.
What could be causing this? The time left to RMA is soon ending.
Would this be a card problem? Some suggested a PSU problem. Checking my system, I found the atievxx.exe was in the "low trusted" settings of Kaspersky and I just now put that in trusted. I have reseated the card, but I believed it was in well to begin with, and an event occurred after reseating it. I only poulled it out a small amount though.
I have run the X2 Terrain conflict rolling demo and 3DMark06 several times each. And those applications put my card under stress far more than anything I have ever done with the card and their results seemed on par. the X2 forums are bad and difficult to search, but I was getting results on 1680x1050 that were better than their last rolling demo at 1024x768 (I believe X2 is cpu bottled anyway, card only spiked to 97% briefly). 3DMark score was 98xx and that is also a cpu reliant test, but scored the same with another 4830.
And this is what I got -
