Alter tables within a .doc

I'm attempting to fill out an application for an advancement at work.

i have two basic questions...

First, what program was used to created these wonderfully useful forms?

Is there a way to override the size of some of these editable areas?

There are professional add ons for formatting forms in Word, but all the basic tools are already there. You can set up your own forms with tables and then adding input fields for people to type into. You can set data validation for the fields as well.

This can also be the foundation for creating PDF forms that can be filled in electronically.
