I wouldnt recommend a 'crash course' book, unless you have great trouble with logic and or maths get a more indepth detail book.

As a student I use a book by larry ullman entitled php and mysql for dynamic web sites this may only be avail in europe tho ...
 
the wrox press books are very good. They explain things in more english terms than technobabble. And have some sort-of-humour in them also. Which makes them more enjoyable to read.

The book No Limit mentioned is also very good but it isn't much of a "learn from scratch" book as more of a "you know the basics, now here are some cool things you might like to try" book.

Sams books are ok, but they aren't the greatest imho.
 
Don't forget the documentation found on php.net and mysql.com - both are great language references and include valuable user comments and discussion. You can download both resources in their entirety.
