Evga 7800 gt rebate question

Since the rebate form asks for the original upc, do I send the one that is on the box or the sticker on the card? Also, is my receipt just going to be a printed invoice from my zipzoomfly order? Just want to make sure I have everything right as MIRs are very picky.
 
i was hoping you would cut out the one on the card and take a picture of the PCB with a hole in it. :D
 
lol, I'm not THAT dumb :p . I would have tried to peel it off. Although, it would make a funny thread, like the dude that cut up his 7800gtx. :eek:
