How does HDR work with PC games these days? Are some PC games designed for HDR and some aren't? Or is HDR in PC games strictly an upscaling sort of thing, kind of like playing an old 480p game on a new screen that gets auto upscaled?

It works the same as on consoles. The game has to be designed for it. Microsoft's Auto HDR works well enough, but it is no replacement for proper luminance data tailor made for the content being rendered.
Pretty much this. Better than it used to be, still a long way from ideal.

How would I be able to tell if a new game has native HDR or not?

I remember HDR not being great in windows. Have things improved? Is there a difference between Windows 10 (my os) and Windows 11?

The only issue HDR ever had was there not being a universal implementation method when Windows 10 came out with support for it. Now you simply toggle it in the Windows display settings and it will work regardless. It has worked that way since Windows 10 20H1.
