I have a gyration mouse remote (more buttons than the one you linked) and I hate it. I was pretty excited when I first got it - ergonomically it's fine, and used it for a while with my HTPC, but mouse control is awkward and annoying. It now sits unused on a shelf. I use compact trackpad / keyboardcombos for my HTPC (as a remote) and as a presentation carryon for my laptop.
I know it's often not possible, but if you can, try before you buy or at least be prepared for an expensive paperweight. Just my two cents.

Not sure how soon you need it or perhaps if you want to make one yourself. A friend of mine makes something like this, but some of the more advanced features are still in development.
