Memory Module Went Bad

I had some memory that just went bad on me, Corsair XMS-512-2700C2 (PC2700) had it oced to:
166Mhz-->200Mhz
2-3-3-6,1T @ 2.8V


(MEMTest86+ test 5 now shows errors even at defualt speeds from 175-345MB range)

lasted for about a month and a half, then some of the memory addresses started to go bad... gonna RMA this stick...

Is it the voltage that cuased the ram to start to fail or the timings or speed.

probably mostly the heat from the extra voltage. how hot does it get while you're running memtest?
 
the way i did it was with dividers, so i had the main cpu bus at 256=2.3Ghz and the ram at 166+46=200~
