Have a local Home Depot/Lowes/Rona?
Get any 6-32 screws over 2" long, and some 6-32 wing nuts...
Works for me!
As for the sticky pad...I used mirror mounting tape, and it worked flawlessly (the foam stuff of course).

It's been awhile from the last time I ordered from them, but it all went fairly quickly and without any problems. Spare parts and fasteners seemed to ship really quick, blocks and the like took like a week or so to ship.
