Yes, it was indeed a problem with the cable box. According to the manual, I needed to set the HDMI audio to PCM. Problem solved!

Yes. I had my ps3 connected to my TV before and sound came out fine.
For some reason when I set it to "TV", audio from the receiver is disabled when I turn it off but I'm still not getting any sound from the TV--just video.

I would definitely recommend Precision Acoustics' HD45 tower speakers. They sell them at Best Buy here in Canada but I'm not sure if they are available in the States. A friend of mine has a pair of those + center. Just amazing sounding speakers for the price. They are currently at $300/pair.

I tried to hook up some speakers (POLK AUDIO RTi5) to my receiver (DENON AVR-591) using 16gauge wires and MONSTER banana plugs,,, but guess what? the banana plugs won't fit into my receiver! They are just too big. Do banana...
